Agreement:
- I understand a Rescue representative may visit my home for a home inspection before my adoption
application is approved. - I understand that if I am approved for adoption, I will also need to carefully read the “Adoption Agreement,” which is a separate document from this “Adoption Application.”

and are truthful. I make this statement under penalty of perjury under the laws of the state(s) of
Texas.
Information:
The adoption fee is $300.00. This adoption fee includes transport costs and the cost of vetting which includes: deworming, spay/neuter, age-appropriate vaccinations, and Rabies shots. Your new dog will need to be kept on heart worm prevention for life.
Adoption Fees:
Puppies 6 Months and Younger:
The adoption fee is $300. The adoption fee includes all age-appropriate vaccinations (spay/neuter, microchip, health certificate) and the transport fee covers transport to the Pacific Northwest on the next available date).
*If puppy is not old enough for spay/neuter, STAR will enter into a foster to adopt contract with the adoptees. Legal ownership of the animal will not be able to be completed until the spay or neutering is complete. All females are spayed by the age of 12 weeks, males will be neutered once the testicles have dropped (or depending on the advice of our veterinarian)
Puppies/Dogs 7 Months and Older:
Adoption Fee of $300. The adoption fee includes all age-appropriate vaccinations (spay/neuter, microchip, health certificate) and the transport fee covers transport to the Pacific Northwest on the next available date).
*** Adoption fees may be waived or discounted.
*Full Vetting includes:
-Altering (spay/neuter)
-Microchip
-Shots: AGE APPROPRIATE vaccinations
-Heartworm Test and Monthly Heartworm Prevention through adoption
